New Patient Registration

All new patients registering with the Practice are requested to complete a questionnaire about their medical history, and provide proof of identity, i.e. a passport and a utility bill with your address. If you are taking medication please remember you will need to see the Doctor before we can issue repeat prescriptions.

How to Register

The Practice has registration packs which can be collected from from the surgery between 8am - 6.30pm from the front desks.

If you decide you wish to register with the Practice after reading the Practice Leaflet then you must return to the Health Centre between 9am - 3pm Monday-Friday only with the forms completed and the documents required as stated in the pack.

Our registration clerk will then take photocopies of your documents.  If there is any problem with your registration the practice will notify you within 2 weeks.

Please make sure you complete the forms correctly especially medical history and details of previous GP. 

As we are an extremely busy Practice we have to ask you to return at lunch-time so our Receptionist and Registration Staff has time to process your registration.  This is impossible to do during busy surgery times.
